User Consent and Acknowledgement Form

By selecting "I Agree", registering an account, activating a subscription, purchasing a Sakhi Wearable Device, or otherwise using the Sakhi Platform, I voluntarily provide my informed consent and acknowledge the following:

1. Consent to Terms of Use and Privacy Policy

I confirm that I have read and understood:

• Sakhi's Terms of Use;
• Sakhi's Privacy Policy;
• This User Consent and Acknowledgement Form; and
• All other policies and notices made available through the Platform.

I agree to be bound by the foregoing documents and consent to the collection, processing, storage, use and sharing of my information as described therein.

2. Consent to Collection and Processing of Personal Information

I consent to Sakhi collecting, processing, storing and using my personal information for:

• Creating and maintaining my account;
• Providing safety services;
• Operating SOS functionality;
• Enabling communication with Protectors;
• Enabling emergency escalation features;
• Providing Safety Grid functionality;
• Processing subscriptions, purchases and payments;
• Customer support and grievance handling;
• Fraud prevention, misuse detection and security monitoring;
• Compliance with legal and regulatory obligations;
• Improving and developing Platform features and services; and
• Any other purpose described in Sakhi's Privacy Policy, Terms of Use, this User Consent and Acknowledgement Form, and other applicable Platform policies, to the extent permitted under applicable law and based on my consent where such consent is required.

3. Consent to Device Permissions

I understand that the following permissions may be required for operation of Sakhi's safety features:

• Location Coordinates Access: For activation and delivery of SOS alerts, live location sharing during an active SOS or safety event, sharing location with trusted contacts (Protectors), emergency escalation, Safety Grid insights and risk awareness, displaying safety zones and nearby locations, route-based and location-based safety suggestions, and improving the effectiveness and reliability of Platform services.
• Microphone Access: For SOS audio recording functionality, audio evidence capture during safety events and emergency communication features.
• Camera Access: For capturing photographs or video during safety events, submission of evidence or safety-related information, verification of certain user actions, and submission of photographic evidence for warranty (replacement or repair) requests related to the Sakhi Wearable Device.
• Notification Permission: For SOS alerts, Protector notifications, emergency communications, account security notifications, service-related alerts, subscription and billing notifications and safety-related communications.
• SMS Permission: For sending/receiving SOS alerts by SMS where internet connectivity is unavailable or unreliable, sending Protector invitations, OTP-based account verification, emergency communication functionality and delivery of safety-related communications.
• Background App Activity: For maintaining continuous safety functionality, supporting live location updates during active SOS events, ensuring proper operation of emergency features and enabling safety services when the App is not actively open.
• Lock Screen SOS and Shortcut Access: For enabling emergency shortcuts, facilitating quick access to safety functions, supporting emergency workflows, enabling rapid SOS activation and reducing the time required to trigger safety features.
• Bluetooth and Nearby Device Access: For pairing and operation of the Sakhi Wearable Device, transmitting SOS signals from the Wearable Device to the paired phone, monitoring device connectivity, maintaining wearable functionality and enabling Bluetooth-based safety features.
• Phone Call Functionality: For emergency call functionality, emergency escalation procedures, communication with authorities where applicable and safety-related calling features.
• Contacts Access: For selecting and inviting Protectors, enabling emergency contact functionality and facilitating safety communications.
• Device, Network and Diagnostic Information Access: For improving Platform performance, troubleshooting technical issues, monitoring service reliability, preventing misuse and fraud and maintaining Platform security.
• Wearable Device Status, Connectivity and Battery Information: For monitoring device connectivity, displaying battery status, ensuring device functionality, assisting with troubleshooting and improving reliability of safety features.

4. Communication Consent

I consent to receiving communications from Sakhi through:

• SMS;
• Email;
• Push notifications;
• In-App messages; and
• Phone calls.

5. Acknowledgement of Limitations

I acknowledge that Sakhi is a technology-assisted safety platform and not an emergency service provider. I understand that no technology platform, mobile application or wearable device can guarantee personal safety. I acknowledge that Sakhi does not guarantee:

• Delivery of every alert;
• Accuracy of every location update;
• Response by Protectors;
• Response by nearby Sakhi Users;
• Response by police authorities or emergency services;
• Prevention of crime, injury, harm or loss.

I understand that functionality may be affected by internet connectivity, GPS accuracy, mobile network availability, Bluetooth connectivity, battery levels, device settings, operating system restrictions and third-party service availability.

Declaration

1. I confirm that I am at least eighteen (18) years of age.
2. I confirm that the information provided by me is accurate and complete.
3. I confirm that I have thoroughly read, reviewed, understood and had the opportunity to peruse the Privacy Policy, Terms of Use, this User Consent and Acknowledgement Form, and all allied policies, disclosures, notices, guidelines and other documents made available through the Sakhi Website, Sakhi App and/or Platform. I acknowledge that I understand the nature of the services provided by Sakhi, the permissions requested, and the manner in which my information may be collected, processed, used and shared, and I voluntarily agree to be bound by such documents.
4. I voluntarily provide this consent and understand the purposes for which my information will be collected, processed, used, stored and shared.
5. I understand that I may withdraw consent in accordance with the Terms of Use and Privacy Policy, subject to the consequences described therein.
6. I understand and acknowledge that by submitting this User Consent and Acknowledgement Form, I am voluntarily granting the consents, permissions, authorisations and acknowledgements contained herein, and that I shall remain bound by this Form, the Privacy Policy, the Terms of Use and all other allied policies made available by Sakhi.
